Summary for OEA's Central Plan

Insurance Period: September 1, 2008 - August 31, 2009

Premium rate: $99.00 per month (pay every 6 months)

Policy Number: 2008-000513-01
Plan Name: Preferred Providers Network (PPO)

Maximum Benefits: up to $750,000 for accident and sickness, as well as Medical evacuation and Repatriation of Remains Benefits, all of which meet or exceed current J-1 visa requirements

Covered Medical Expenses: 100% in-network (including Student Health Center) and 80% out-of-network.

Outpatient Prescription Medications, with a copay of $10 for generic drugs and $20 for brand name drugs.

For 2008-2009 policy year, all blood tests related to the physical exam will be covered during an annual physical exam

Moreover, students may access 2008/2009 Insurance Brochure for Thai Scholars, 2008/2009 Insurance Brocure for Dependents, update contact information, and print a temporary ID Card for Thai Scholars and temporary ID Card for Dependents by going to UnitedHealthcare Student Insurance Plan and select Royal Thai Embassy from the drop-down list (the information will be available after September 01, 2008).

*Students must update contact information with OEA and Insurance Company separately.*

For those of you who need to visit a doctor or hospital but have not received ID Card, 
please contact Customer Service Department at 1-888-224-4875.

Additional Service: For any Thai scholars who has requested to be insured with Insurance Company provided by OEA could apply for Dental plan at your own cost. More detail.......
